- OS specific instructions moved to separate help files, so \clearpage can
  be used in Authoring Manual to position floating images with the text to
  which they refer.

- Consistent title capitalization in table of contents for author manual and
  course coordination manual.
- Remove TEXitemgroup_attribute.tex (TEXitemgroupwidth_attribute.tex which
  is a duplicate is retalned).

- Eliminate term 'Course Author' in favor of 'Content Author'.

Version 1.21 was inadvertently committed with DOS line endings. This is the same as 1.21 except line endings are linux.

This should be all the help information necessary for the next stable
release.

Added index tags, fixed irrelevant bug in author.manual.texxml, created
spreadsheet.manual.texxml .

Added help to the Publishing page as a trial run for the help system.
Required splitting the .tex pages a bit more.

This commit implements the .tex-based online help system in LON-CAPA. It
may be necessary to manually run loncom/build/help_graphics_converter.pl
and doc/help/rebuildLabelHash.pl after a cvs update, and
loncapa_apache.conf may need to be manually copied to /etc. (The
loncapafiles.lpml has been updated.)

After merging this, the URL http://[loncapahost]/adm/help/Foils.hlp
should display a help file.

Also, some *very* out-of-date files have been removed from the /doc
directory.
