Added support for graph legends/keys

Added handling of y1_label and y2_label for multi axis plots.

Added some POD, comments, and changed the  manner in which the maximum
y value is passed in to be truer to the GD::Graph name of the parameter.

Added support for two axes graph and commented out some debugging code.

Complete rewrite of how graph.png works and of the interface routines for
it in loncommon.pm.  Added &DrawXYGraph which occasionally works.

Various changes.  No longer modify look of plot based on title.

New settings on plots:
    x_label_position = 0.5     center the label (sort of) instead of
                               having the label be on the right
    x_tick_offset              Gives control over where it starts counting
                               So now we count 5,10,15,... instead of
                               1,6,11,....

To fix bug #1152 the foloowing is done:
If the number of problems is more than or wqual to 100 then ther x-range would be 100, otherwise the range would be the number of problems.

To fix bug #1052
a) If the number of problems involved is less than 100
    it would be better to change the plotting to
    display "number of problems correct" on the x-axis
    and the range would be from 0 to 99
b) If the number of problems is > or = 100
    Plot the % leave as it is currently.

This version trierd to fix bug #1020 and 1052.
The lonpercentage.pm graph works better and meaningful.
The width between x-labels are 5 among 100 entries.
The whole width of garph is 800 pixel.
The bin between y_labels iS also 5.

Fixed bug #1020.
It works for showing the graph for lonpercentage correctly.
