Using bubblesheets to record grades for handgraded exams

     i.e., 5 for 5 bubbles per line, 10 for 10 bubbles per line etc.)

  - New routine: &hand_bubble_option() determines if sequence to be graded
    contains response types which are handgraded, and offers radio buttons to
    choose score for last bubble.

  - &repetition() routine can be called in scalar context of in array context.
    scalar: returns number of lines to encode weight
    array: returns (number of lines to encode weight, bubbles/line).
chemresponse.pm imageresponse.pm caparesponse/caparesponse.pm
  - &response::repetition() now explicitly called in scalar context.
Work in progress.
  (Corresponding option needed in lonprintout.pm to set
   correct number of bubble rows for handgraded questions with
   weight >= bubbles per row).

- New awards for surveys:
 ANONYMOUS ANONYMOUS_CREDIT SUBMITTED_CREDIT
  - given for question types: anonsurvey, anonsurvey_cred, survey_cred.
    - anonsurvey -- course personnel can not view both identity of submitter
                    and submission details.
    - anonsurveycred -- same anonymity, but submitter receives 'awarded' for
                        submission
    - surveycred - submitter receives 'awarded' for submission to survey.

- Restore pre-2.7.1 functionality: "Show Your Last Answer" button displayed if after answer date, but no jmeanswer (for display of computer's answer) provided in problem.
- Remove code no longer used from start_organicresponse() following changes in 1.79 and 1.80 which replaced "Draw Molecule" button before input textbox with pencil icon after textbox.
- Change wording in CSTR for textbox entry for JME string of the answer, now pencil icon is used instead of "Draw Molecule" button.

Change arguments accepted by lonnet::appenv() to:
1. reference to hash
2. (optional) reference to array.

Change all instances where lonnet::appenv() is called to replace first argument passed (originally a hash) to a reference to the hash.

- Modify lonnet.pm rev 1.35 code intended to prevent "dangerous" modifications of the environment, so it will actually do this.
- Allow these modifications to be made in certain instances:
   DC switching to adhoc role
   CC switching to a different course role in the current course.
   user self-enrolling as a student

- imageresponse, reactionresponse and organicresponse behave the same way as essayresponse.  When used in an exam the number of bubble lines per problem part depends on the weight assigned to the part.
- Part Ids for reactionresponse and organicresponse are not contained in $analysis{'parts'} from lonnet::ssi, so they need to be retrieved from the resource object.

When the target is 'analyze', include the responsetype of the current item in the lonhomework::analyze hash, for use later on, e.g., for scantron grading.

BZ 4074  Ensure the bubble lines chunk of the analysis hash gets
keyed by the concatenation of partid.resopnseid.

Bug #3449, Safari workaround for document.open

Besides being wrong, this did not solve anything.

Trying to fix Bug #3449. The previous syntax was definitely wrong, not sure
if this works.
