Minor change and added help file for Score Upload Form.

A \textbf{Score Upload Form} is a page in LON-CAPA that stands in for a score in an
assignment external to LON-CAPA. You can insert a Score Upload Form into
your course, and use it to enter student's scores for an exam, project, or
any other assignment. The easiest way to do this is via the \textbf{PGRD} score
upload screen, which allows you to upload a CSV file containing student's
scores.
