Fix wording typos

- Both cloners and description need to be stored in nohist_courseids.db
  for domain
- Rename input item to from can_clone to cloners for consistency.

 - Update entry in nohist_courseids.db when course initialization helper is used to update course description.

- Update informaton about options for date shift when cloning a course.

- Replace PARMS with name of corresponding Main Menu item since the majority of users do not use the Remote Control.
- Set Course Environment renamed Course Configuration.

    Don't use unescaped "&"
- Better separation of content and style
    Moved quotation marks and bold tags from mt text to script code

  (This was reported by CCs to be confusing)

Bug 2213 (localize helpers) is finally done.

Tell the user they are going to the course initialization screen if the
course is cloned.

Bug 679: Need to be able to disable chatroom.
   Added plc (permission for live chat).
   lonchat.pm and lonchatfetch.pm now both call &lonnet::allowed('plc',...)
   lonparmset.pm, loncreatecourse.pm, and course.initialization.helper
       now all allow setting $ENV{'course.'.$courseid.'.plc.roles.deny'}
       lonparmset.pm also allows the user to set
       $ENV{'course.'.$courseid.'.plc.users.deny'}
   lonmenu.pm was modified to allow us to require the user both be in
       a course AND have permissions.
   mydesk.tab, roles.tab, and rolesplain.tab were modified to include the
       new role.

Fix a speling eror; fix bug 1969 as a result.

Remove some garbage out of the helper.

Remove a little white lie; they're now taken to the main menu, not the
NAV screen.

A voodoo fix attempt for bug 1757. (See comments on that bug entry.)

When they run the course.initialization helper the first time, don't run
it again. What an idea!

Now that we have a "menu" screen, we can use that as a "default" screen
for the course when we want to do things like "leave this helper",
instead of just going out to an essentially random thing like navmaps.

Make HTML discussion work, added some text, added the ability to specify
course feedback address or addresses, added reinitialization support to
the <final> tag, added a place to optionally change the description of
the course.

Require the users of the parameter helper and course initialization helper
to have the 'opa' priviledge of setting parameters.
