Stopped wrapping in Time-Limit settings frame

<select> elements and their labels were grouped into <span>'s with
style="white-space:nowrap".  It was previously possible for the label to
wrap to the next line, making it unclear what was being set.

The following functions were changed to return strings instead of writing
directly to the page.  The results can now be embedded within html strings.

Bug #3465: radio-button-based parameter setting on Safari.

Safari does not honor "onChange" event for radio buttons, wants "onClick"
instead. Now firing both events.

Needs documentation of what each type does.

Was unsure of the possible values for "Exam Type" and decided that those
are "online" and "checkout" for now.

Good old bug 610 - just don't mention things like "start" and "end" ...

A little less color choices render a whole lot faster - but I am sure I am
missing somebody's favorite color now, which justifies another war.
